Program Purpose / Objectives
The vast majority of patients treated in Palliative care experience unpleasant physical, psychological, or spiritual reactions that may interfere with a person’s capacity to cope with their illness or treatment. T he management of stress of this type is clinically challenging, as identifying and treating the underlying cause of symptoms can be difficult. This activity will address common issues in those domains of care.
Topics:
  1. When is the appropriate time to initiate palliative care discussions in an outpatient setting?
  2. Identifying patients in need of palliative care assessment in the hospital setting
  3. Palliative care for chronic obstructive pulmonary disease patients, including case study
  4. Palliative care for congestive heart failure patients, including case study
  5. Aspects of pain in patients with advanced disease
  6. The main non-pain symptoms which are prevalent in advanced diseases
  7. Incorporating spirituality into patient care in regards to palliative care, hospice, and time of death
  8. Communication issues with patients and their families
  9. Things they never told you about grief
  10. Sorting out confusion
  11. Cognitive impairment (when to worry about memory)
  12. Treating depression in palliative care
  13. They say I’m getting crispy around the edges: Burnout

Objectives:
At the conclusion of this activity, participants should be able to:
  1. Apply screening tools which can be used in an outpatient setting
  2. Determine which patients would benefit the most from palliative care and then apply tools to improve their care
  3. Apply a scoring scale for prognostication purposes of COPD patients
  4. Apply a scoring scale for prognostication purposes of CHF patients
  5. Provide appropriate pain relief while trying to identify the best balance in regards to side effects
  6. Be able to prepare and help patients’ families from the time of initial assessment and onward to time of death
  7. Incorporate spiritual care tools and discuss these aspects with patients and families
  8. Implement practical strategies for working with angry and dependent family members
  9. Effectively manage family members with a predisposition toward challenging behaviors
  10. Review fundamental components of effective communication with patients and their families
  11. Discuss potential barriers to effective communication in palliative care
  12. Describe common reactions to loss
  13. Differentiate between uncomplicated and complicated grief
  14. Distinguish between grief and depression
  15. Effectively distinguish between the major causes of confusion in elderly patients
  16. Differentiate mile cognitive impairment (MCI) from normal aging and dementia
  17. Prescribe medication to slow progression to dementia in a patient diagnosed with MCI
  18. Advise patients on nonpharmacologic interventions to prevent or slow cognitive decline
  19. Differentiate depression from delirium, dementia, adjustment disorders, and grief
  20. Rationally choose medications for the management of depression at the end of life
  21. Self-monitor for symptoms of burnout
  22. Employ personal and organizational interventions to address the problem of burnout
  23. Identify causes of unusually high rates of burnout and depression among physicians
